The Arizona Alliance for Chronic Care is comprised of 19 Arizona-based organizations united by the challenges of living with chronic disease have formed the Arizona Alliance for Chronic Care (AACC). The first of its kind in the state, the AACC is a new unifying voice for individuals and children who have been diagnosed with illnesses that are permanently altering their lives.
Defined by the U.S. National Center for Health Statistics, chronic disease is an illness that lasts three months or more, it often limits activity and may require ongoing care. It is estimated that more than 125 million Americans have at least one chronic condition, such as diabetes, cancer, heart disease, chronic pain and Alzheimer’s. Nearly half as many have more than one chronic condition.
According to the 2009 Arizona Health Status & Vital Statistics, chronic disease accounts for 12 of the 15 leading causes of deaths for the state of Arizona. They are the most prevalent, costly, and preventable of all health problems. Early detection and treatment of chronic diseases can help reduce its complications.
The AACC seeks to empower the chronic care community to take action to improve their quality of life; identify, prioritize and drive awareness to key issues, be recognized as chronic care experts and collaborate with key decision-makers to find workable solutions.
Members of the AACC represent more than 19 prominent Arizona-based organizations that advocate for a variety of chronic illnesses, from fibromyalgia, autism, juvenile diabetes, arthritis, pain, spinal cord injuries, mental health issues, among many more.
